Both forms clarify that I am not a medical doctor and do not offer medical treatment. I am a Doctor of Naturopathy and I follow the principles of Naturopathic Medicine (more info). I believe that mainstream medicine is very important for surgeries, emergencies, and trauma. But, for maintaining our optimal every day health, I believe Naturopathic Medicine is the way of the future.

Consent Form #1 clarifies that I am not a medical doctor, and you understand that under the constitution of the United States, you have the consitutional right to decide how you wish to care for the health of your body.

Consent Form #2 gives me permission to do AVATRAR Electrodermal Screening (EAV) using saliva swabs from your mouth. This process helps me to determine which nutritional supplements and homeopathics are best suited for your specific nutritional needs.
